- Health Assessment Checklist
- Volunteer Request to Access Restricted Information (If applicable)
- HIPAA & Privacy Training: PRV800
- General Compliance Training: PBC810
- FERPA Training: PRV802 (If applicable)
- Copy of government-issued ID, i.e., Driver’s License (if Foreign National, then Gator1 Card or passport)
- Work authorization documentation is required for foreign nationals: i.e., forms I-94, I-20, DS-2019, and/or EAD (front and back).
- Background Screenings will be conducted on all volunteer applicants upon successful completion of all required forms.
Additional Requirements
- Supervisor completes additional trainings
- Youth Protection Training: YCSS00
- Mandatory Reporting of Child Abuse: YCS100
- Supervisor completes a Level 2 DCF Background Check
- Supervisor registers the activity with UF Youth Compliance.
- Activity Involving a Minor Form, based on the activity and time of year
- General Activities
- Laboratory Activities
- Summer Activities: Requires the current year’s form
- Parent/Guardian Authorization Form
- Required Consent Form for Minors (for background check)
Activities must be coordinated in a way that eliminates one-on-one interactions and ensures that adequate staff-to-participant ratios are in place at all times.
All Activities involving a minor must adhere to the Youth Compliance Policy.
